You have a problem controlling your Set Top Box which is supported by TiVo, and you are using the recommended IR codes.

For the most reliable control of your Set Top Box it is recommended that you use the external IR wands rather than TiVo's front IR blaster. Make sure the front IR blaster is turned off in TiVo's STB Control settings. Use only one of the IR wands and hide the other one out of sight - ideally cover the emitter with black tape. Try different positions for the IR wand to see which works best - on some models the position is crucial for reliable operation. Try the wand two inches in front of your Set Top Box IR receive window pointing either up or down.
Ensure that there are no remote controls operating in your room. Do not overlook this check - many users have had issues only to discover that one of their remotes was stuck under a cushion with a button pressed down. Make sure all of your remotes are accounted for!
If you have a wireless video sender, or a remote control extender in the room, then try turning these off. They can often interfere with TiVo's control of the Set Top Box. Some models have a sensitivity control which can be tweaked to prevent interference.
Try turning off all of the lights in the room. Some types of lights can cause TiVo's control of your Set Top Box to fail. If its daytime try pulling on the curtains to cut out sunlight.
Personal Computers and Laptops that have an InfraRed port are often configured by default to automatically search for InfraRed devices to connect to. In doing so they emit bursts of InfraRed every few seconds. This may interefere with TiVo's operation. Either disable the InfraRed port, or ensure it is well away from the TiVo (and pointing away from it).
Reboot both TiVo and your Set Top Box (switch them both off and then back on again). Retest to see if you have more reliable operation.
Try building an IR Fort. An IR Fort is just a cover around the front of your Set Top Box and TiVo's IR wand so that no external light (such as from other remotes) can penetrate the fort. Only TiVo's IR wand can throw light onto the Set Top Box IR receptor. You can try using your hands or a cardboard box as a temporary measure. To check your fort is working try using your Set Top Box remote control - it shouldn't be able to control the Set Top Box.
If you are a digital cable user and have a Pace 1000 or 2000 box, or you have a Pace 4000 box and are in a Telewest or ex-Cable&Wireless area, then you probably need a special dongle from customer services for reliable control of your Set Top Box.
